INTRODUCTION: Nasolabial cyst is a benign lesion arising in the soft tissues adjacent to the maxillary alveolar process before, being unilateral in 90% of cases arising from the 4th decade of life, predominantly in females, and its slow development and asymptomatic and may remain unnoticed, except when infected or when they reach large proportions.
CASE REPORT: A 42 year old female with clinical bilateral nasal obstruction and progressive bulging of the face 2 years ago. Computed tomography of the sinuses revealed a cystic lesion with massive displacement of the nasal septum and medial wall of the maxillary sinus without bone infiltration, resection was indicated using access sublabially (degloving). Histopathological examination of the surgical specimen identified lesions compatible with nasolabial cyst, and the evolution with good quality and respiratory satisfactory cosmetic result.
DISCUSSION: The interest in the description of this case is shown in the rarity with which the disease develops in these dimensions, assuming large proportions with facial deformity and nasal obstruction. Resection was performed sublabially (degloving), with careful dissection and complete lesion without damage to adjacent structures, with satisfactory functional and aesthetic result.
