Kienböck’s disease with no cubital variance

Kienböck disease is defined by avascular necrosis of the lunate and affects preferably men, between 20 to 40 years old. Pathophysiology is multifactorial. Clinically, patients present with pain and decreased muscle strength, progressing to carpal instability and degenerative arthritis. Radiography is a key diagnostic test, but MRI is particularly important to the diagnostic in the early stages. Most accepted classification is the Lichtman and used to define clinical conduct and the most appropriate surgical procedure. We report a casa where patient was 46 years old when started the symptoms and the final diagnosis, kienböck’s disease, happened ten years later, diagnosis and treatment were negative factors to prognosis and consequences of the functional limitation.
