The scafocapitate syndrome, also known as Fenton’s syndrome, is a diagnosis that defies many clinicians because it is a rare occurrence. The objective of this study is to report the case of a patient diagnosed with Fenton’s syndrome. A 49-year-old patient suffered a motorcycle fall and left wrist trauma, evolving with pain, edema and deformity. Radiography and tomography of the left wrist evidenced dislocation of the lunate and fracture-avulsion of the bone fragment of the intra-articular capitate. An open reduction and bone fixation was performed, preserving the strength and sensitivity of the affected areas. Because it is an uncommon syndrome, it often goes unnoticed by care, which causes delayed diagnosis and impaired prognosis. The diagnosis and appropriate treatment of Fenton’s syndrome are important for the emergency orthopedic surgeon and traumatologist to avoid carpal instability in the future.
